long absence

time has really passed since I last blogged...

Been more than a month since I came back from europe and i'll be putting up the very much overdued photos soon.. at the moment, my time is out of my control, with everyone seeming to want a piece of my time, haha, or am i just abit paranoid. DOn't really wanna talk about it coz i have loads and loads to do and settle...

Anyway, here's a case for those who may want to play a guessing game

patient is a 21 year old student, currently studying in japan, with no known medical illness. Admitted for pain in the right iliac fossa for 2 days The pain was described as throbbing in nature and more painful when pressed. The first episode took place 4 months ago in japan where he was diagnosed wit appendicitis. He defaulted surgery as it was too expensive and he said the pain dissapeared several days later. He carried on his studies and the pain came and went on and off without a predictable duration or time. when he cameback to malaysia, the pain became worse and he was admitted to the hospital.

o/e clinical stats were normal, not anemic or dehydrated. Tenderness on Mcburney's point , Rovsing's sign possitive.

Ix showed increased in total white, differential count showed neutraphilia. All other parameters were normal

1. What is the most probable diagnosis and why? differentials?

2. what organ is showed below? and what does it show?
